Service Summary:The Trafford Community Neuro-Rehabilitation Team is a well-established multidisciplinary team with extensive skills and knowledge. It is split into two pathways; stroke and neurological conditions. It comprises of occupational therapists, physiotherapists, speech and language therapist, psychologist, specialist nurses (stroke, complex neuro and parkinson's disease), dietitian, and therapy technician. The team see anybody over the age of 16 with a neurological diagnosis.The role will be based at Partington Health Centre, the post holder will be expected to work in the community across the Trafford area.Role Summary:We are looking for an enthusiastic Speech and Language Therapist with experience of working with adults with neurological conditions. The post holder will be based within the stroke pathway of the team, supporting, as where necessary, the neuro pathway caseload.You will be part of a well-established, innovative team who are committed to providing a high quality, patient focussed service. There are regular opportunities for MDT in-service training, joint working and professional support and supervision. Key Responsibilities of the Post: You will be responsible for:- Independent management of the assessment, diagnosis and treatment of clients with a stroke- Participation in weekly MDT meetings- Liaison with third party organisations- Delivery of service improvementKey Requirements of the Person Specification:? Appropriate relevant post grad experience working in a community setting with a neurological caseload ? Evidence of working with people with complex communication and swallowing disorders and their carers? Evidence of working in a multi-disciplinary team? Evidence of working with clients with specialist needs? Supervision of junior staff/ students? Clinical Audit and standard setting?
