The successful candidates will join teams across Cambridgeshire and Peterborough who have already trialled this way of working.The Case Management approach and integrated model of proactive care is achieved through a broader multi-agency and multi-disciplinary approach that is systematic and well-coordinated. People who are identified as potentially benefiting from such an approach will be supported through the model of Case Management by a team of workers including GP?s, voluntary sector, Community Nurses, Mental Health Workers, Adult Social Care Workers, Case Managers, Occupational Therapists and Physiotherapists. Partner agencies will build on current efforts but have at their heart a population approach to multi-disciplinary/multi-agency working and Case Management.The Case Managers will form part of the present Neighbourhood teams and work closely with colleagues from other agencies to benefit identified patients to stay well and independent for longer.The ideal candidates will have a strong team work ethic, sense of accountability, accompanied by an ability to work under considerable pressure at times. The post is open to professionals with the right attributes to enhance a team, we are seeking team members who demonstrate a commitment to improving our service.Cambridgeshire &Peterborough NHS Foundation Trust CPFT is launching an exciting new model of care built around the patient in the community.
