Do you have an interest in or a passion for developing and delivering quality community based health care, improving older people?s health and wellbeing, promoting active ageing, safer independence and adding ?life? to years?If so, this is an exciting opportunity to be part of the Manchester health and social care transformation and join our rapidly expanding and evolving Community Falls Service within South Manchester.The Community Falls service is being expanded and transformed to significantly improve the responsiveness and access to evidence based and innovative community based falls and bone health management and prevention.Multifactorial falls and bone health assessments, management, treatment, exercise will be predominantly home-based but may also delivered in other community locations/ clinics A key emphasis of the post will also be around active case-finding, effective signposting health promotion and delivering a strong educational service around falls and bone health management for colleagues in the community and primary care.We are looking for an enthusiastic and innovative Specialist Occupational Therapist with experience in falls assessment and rehabilitation in older person?s health, who is motivated to both support the development of the occupational therapy provision and the wider objectives of the falls service. Experience of working independently in a community setting and/or of working with a community rehabilitation team is essential. The team will be part of the Integrated Community Rehabilitation Service and will work alongside other experienced and specialist physiotherapists, occupational therapists, falls nurse, podiatrist, pharmacist, therapy assistants.The candidate will demonstrate a good understanding of the multifactorial causes of falls, fragility fractures and frailty, managing cases both independently and as part of a team. The post holder will work within professional standards and clinical guidelines to promote best practice and will undertake all aspects of clinical duties, service planning, development and evaluation, as an autonomous practitioner. The post holder will build up and maintain key relationships with relevant and referring services across the evolving rehabilitation pathways and patient journeys, in Manchester and will be responsible for supervision of junior staff, rehabilitation assistants and students.The post is full time 37.5 hours week with an expectation to work flexibility to support the Integrated Community Rehabilitation Service as needed. Currently the post is delivered mainly over Monday to Friday with some weekends/Bank Holidays.The post holder will be expected to contribute to departmental rotas as necessarySupervision, personal development reviews and professional development opportunities are available and CPD is supported and actively encouraged.
