Occupational Therapist Band 6Temporary contract April 1st 2014-March 31st 2015 Post Title: Temporary Occupational Therapist- Band 6Service/Ward/Department: Community Occupational TherapyBorough: OldhamBand &salary scale: Band 6 Hours of work: 37.5Service/Ward/Department Summary:The Community Occupational Therapy Service is centrally based within Oldham town centre. The post holder will be responsible for providing a community equipment, adaptations, re-housing and rehabilitation occupational therapy service to clients with a physical disability. They may also have conditions including mental health problems, learning disabilities or behavioural disorders which may affect their physical functioning. The post holder will work in line with the agreed objectives of OT Service/Pennine Care NHS Foundation Trust and partners. Role Summary:To work as part of an established team providing assessment and treatment to patients in a community setting and being responsible for your own clinical caseload. OT intervention consists of:-* Promoting independent living and wellbeing through individualised assessments which may lead to the provision of assistive technology (equipment) or environmental adaptations to the home, therefore reducing avoidable admissions to hospitals/other residential care settings and reducing cost of home care packages.* Specialist assessments for seating and manual handling including training informal and formal carers in the safe use of any equipment provided.* Advice to clients and other agencies on re-housing options and designing new builds/refurbishments of housing for people with a disability to reduce impact need for major adaptations.* Independent mobility assessments for the Blue Badge scheme (managed by OMBC).* Specialist OT assessment and treatment planning as part of OMBC reablement scheme including a reablement assessment unit at Medlock Court.The post holder will actively liaise with other services in the community, primary and secondary care to ensure the necessary support is available.The post holder will provide support/supervision/training to peers and junior Occupational Therapy staff as required.There may be a requirement to work flexibly which may include evenings and weekends.Key Responsibilities of the Post: * To assess and treat patients and develop, implement, evaluate and modify individualised treatment programmes, ensuring appropriate pathways of care and communication via liaison and referral to other agencies as required.* To carry a caseload of patients that present with acute or chronic conditions * To participate in appropriate meetings e.g. MDT?s* To ensure the accurate documentation of occupational therapy notes within a multidisciplinary team Key Requirements of the Person Specification:* Degree/diploma in Occupational Therapy* Health and Care Professions Council Registration* Experience in treating patients with a variety of conditions* Able to work flexibly and respond to service requirements* Use or access to a car or suitable means of transport.
