Buckinghamshire NHS Trust is delighted to announce new opportunities for full-time or part-time Community Healthcare Assistants (Band 3) in the Rapid Response &Intermediate Care team. We have vacancies in our Amersham and Chalfont St Peter teams currently but will work with the right candidate to support transfer to one of the other 6 localities across the county These posts involve working with Therapy and Nursing clinicians to provide Rapid Response assessments, rehabilitation, reablement &intermediate care to patients in their home environment. The post holder will work alongside colleagues and with patients to empower and enable their recovery from illness or injury, maximise their independence, and provide care support to those at the end of life.In order to respond to the healthcare needs of the local population, the community teams work shift patterns across a 24 hour period (with a dedicated night team) 7 days a week ensuring the delivery of a service which can respond to the patients? needs as and when required. The successful candidates will be expected to work shift patterns routinely to cover the service hours which are between 8am-9pm with regular rosteredweekend working.Applicants must have previous experience working in a health or social care environment and multidisciplinary team working. Applicants must have an NVQ, excellent communication skills and the ability to work with a range of health professionals and informal carers.Applicants must hold a valid driving license and have access to a vehicle for work purposes for this community post.
