Position: Rehabilitation Case Manager Multi TrackLocation: Home BasedHours: Full Time, Permanent  Hours: 37.5 hours per weekSalary: Competitive We have an exciting opportunity for an experienced Rehabilitation Case Manager Multi Track to join our existing Multi-track Rehabilitation Teams at HCML. HCML, a provider of professional rehabilitation services, specialises in providing case management services to the insurance, corporate and legal sectors, assisting injured and ill individuals to recover and return to work or their pre-injury condition. We are passionate about ensuring that everyone who requires rehabilitation in the UK has access to quality and effective, co-ordinated services. We deliver added value to our clients by facilitating both speedy recovery and strong clinical outcomes, including managing appropriate treatment and care. The main purpose of the role involves undertaking Immediate Needs Assessments (INA) at a person?s home or workplace and using your knowledge and experience to assess your clients? needs and build a plan to best position them towards recovery. Following on from the INA you will work closely with your client and all relevant stakeholders including their family, solicitor, employer, treating team and the insurer to help them realise their rehabilitation goals. This exciting role will involve telephone work, travel, face to face visits, and report writing. In return, we will offer you a competitive salary, a generous pension scheme, Health Cash plan and 25 days holiday. Your Skills and Experience To be shortlisted for interview, you must have the following skills and experience:-At least 4 years of clinical/NHS and/or previous case management, insurance, vocational rehabilitation expertise.A recognised clinical qualification in Nursing, Occupational Therapy or Physiotherapy with professional registration/membership.
