Template AdvertPost Title Speech and Language Therapist Service Urgent Community Care Team/ Intermediate Tier Borough Rochdale Band &Salary scale 6 Hours of Work 15 hours per week Permanent/Fixed term PermanentService:The Urgent Community Care Team (UCCT) is based at Callaghan House, Heywood and comprises of a team of nursing staff, physiotherapists, pharmacy technicians, occupational therapists, social workers, mental health nurses, assistant practitioners and advanced nurse practitioners. The team responds to referrals for adults over 65 years who are in a crisis situation, requiring an urgent response from a team of practitioners who have the necessary skills and range of interventions to keep them out of hospital. These clients are either supported by the team at home or in designated nursing home beds. The team also provides step-down care to patients from A&E or medical assessments units.Following a significant investment by our commissioners in Heywood, Middleton and Rochdale CCG we are in a position to move our service from a 5 day to a 7 day service. This service development is part of an overall redesign of community based intermediate care services to work collectively to develop real alternative to hospital admission. The services will provide care at home or as close to home as is possible. Role Summary:The post holder will be required to work as part of an integrated multi-disciplinary team, which provides care and rehabilitation to people in their own homes and in NHS funded beds throughout the borough. As a Speech and Language Therapist you will be required to assess, plan and implement the profession related care plans. The patients will have a wide range of physical health, mental health needs, complex social needs and you will work as part of the UCCT to best meet the individual care needs of each patient.You will need excellent communication and organisational skills, combined with an understanding and interest in working with older people with complex health needs.Key Responsibilities of the Post:The post holder will work within the service to:? Provide Speech and Language assessment and interventions for the client group described.? Management of a caseload of clients with a variety of conditions ? which may include acute, chronic, complex disabling and life-limiting conditions.? To be able to work independently and flexibly as a professional within the UCCT/ Intermediate Tier.? To maintain a flexible approach to enable the development of the UCCT/ Intermediate Tier to develop new ways of working to deliver the care pathways required to deliver care close to home Key Requirements of the Person Specification:? Registered and a member of the RCSLT.? Registered with HCPC. ? Able to fulfil the physical demands of the role subject to reasonable adjustments under the terms of the Disability Discrimination Act 1995.? Use of a car or access to a means of mobility.?
