INTERVIEWS TO BE HELD ON 5th DECEMBER 2017 District Nursing Adult service from 16 years 4 Cluster Community Nursing teams across Oldham Community Services Service operates over 7 days. Core hours 37.5hrs Part Time and Full Time Available Work as part of a wider Health &social care team to provide complex nursing care to adults in their own home and in a variety of settings within the community. Responsible, following training, for the assessment of patients? needs, planning, implementation and evaluation of the nursing care of individuals or a group of patients with senior colleague supervision. Undertake nursing tasks within scope of competency, knowledge and skills. Deliver high quality services to a diverse population in an innovative way. A flexible approach with a strong commitment to providing and developing new ways of working. Responsible within Trust and NMC guidelines for duties undertaken as delegated bySenior Nurse or Manager. Maintain accurate and contemporaneous nursing records. Promote independence 1st Level Nurse with Current registration with the NMC. Means of visiting patients in a variety of community settings on a daily basis, e.g. car driver with daily access to a car. Awareness of recent government directives and the implications for community nursing. Knowledge of other agencies working in the community. Knowledge of the clinical governance system and its effect on current practice. Previous experience or evidence of understanding of community nursing. Awareness of public health initiatives. Excellent communication skills Politically aware Ability to prioritise own workload.
